Financial Assistance Information

If your child is in need of an evaluation or therapy and you would like to apply for financial assistance
to cover the associated fees, please follow the following procedures:

• Schedule an appointment by contacting the Center's Administrative Assistant at (804) 266-6699.
  
  
• Submit your families' most current tax documents.  If you do not file annual taxes, copies of your last
  two pay stubs will be sufficient.
  
  
• Write a letter explaining the reason for your request for financial assistance (i.e. do not have medical
  insurance, services are not a covered benefit, etc.)
  
  
• Provide a detailed  itemized list of monthly expenses.
   

Once you have completed this information, please mail or fax it to the Finance Committee. 
Eligibility is usually determined within two weeks of receipt of paperwork.
